When You Need Time Away From Home

Temporary care provided for people with disabilities, illnesses, dementia or other health problems while their usual caregivers take a break from their usual caregiving responsibilities. Everyone needs a break.

Care Management

Care managers update all stakeholders immediately as client’s status and care requirements change.

Provides an elevated level of supervision.

Care Management includes a minimum of one in-home visit each month, weekly calls to the client and client family, monthly status reports complete with copies of daily care notes. Provides peace of mind.

Personal Care Visit

Double as a check-in for safety and security purposes for those clients who wish to maintain their independence as long as possible.

During the visit, the caregiver will bathe and groom the client; change linens, tidy bathroom and complete chart notes; notifying agency immediately upon notice of change in the client’s status. Approximately 2 hours in length.

The purpose of a Personal Care Visit is to make sure that the client’s personal hygiene is being maintained on a regular basis. These visits are approximately 2 hours in length and can be performed in the morning or evening, and can double as a check-in for safety and security purposes for those clients who wish to maintain their independence as long as possible. During the visit, the caregiver will bathe and groom the client; change linens, tidy bathroom and complete chart notes; notifying agency immediately upon notice of change in the client’s status. Personal Care Visits do not do light housekeeping except for change of bed linens. Personal Care Visits are administered by CNA or HHA certified caregivers.

The role of a Shopping Visit is essentially that of a delivery service and ensures that clients receive the groceries, prescriptions and items they need without their having to leave home. Clients coordinate all activity through the CHC service team who in turn instructs caregivers directly, providing them grocery lists, prescription script and funds to execute the errands. Caregivers of clients with regular Live In and Hourly Caregivers may perform these duties as directed in the care plan, and as requested by clients at the initiation of services. In such circumstances no additional fees apply.

In no case should a client provide cash directly to their caregivers. For the protection of clients, all financial transactions should go through the office. For clients who routinely require grocery shopping or errand running, CHC will maintain a small deposit and CHC then bills for groceries and prescriptions with the weekly regularly scheduled billing, remitting receipts whenever a transaction has occurred on behalf of the client. For a one-time errand, clients will be billed immediately by the Agency for their purchases.

The role of a Skilled Personal Care Attendant is to assist in the daily living needs of non-ambulatory and high-need clients, while seeking to make their day as comfortable and enjoyable as possible. Skilled caregivers have HHA and/or CNA certifications with knowledge of:

Lifting: Hoyer lifts, assisted and non-assisted transfers
Bed-bound: bathing, positioning, feeding, muscle stimulation and blood flow, monitoring skin integrity
Bowel and Bladder: monitor catheters, incontinence and empty bowel/bladder bags
Cardio & Pulmonary: can take and track blood pressure, pulse, heart rate, administer breathing treatments
Diabetes: nutritional and glucose test monitoring
Rehabilitation: range of motion, massage, exercise, walking with cane or walker
Special circumstances: Hospice, Alzheimer’s, Parkinson’s, Dementia, etc.
Secondary duties of Skilled Personal Care Attendants include light housekeeping, meal preparation, exercise, and companionship as determined feasible according to the care plan provided. All our Caregivers are required to follow care plan instructions and maintain chart notes regarding daily activity, nutrition, medications, general health and emotional well being.

The role of a Companion is to assist in the daily living needs of ambulatory clients while seeking to make their day more enjoyable and interesting. Specific duties include light housekeeping, meal preparation, exercise, companionship and running errands & appointments. Companion Caregivers are required to follow care plan instructions and maintain chart notes regarding daily activity, nutrition, medications, general health and emotional well-being.
